Baby Acne, Heat Rash or Eczema?

Question

I know baby acne is totally normal but starting to worry this may be something more?
LO is almost 4 weeks.
Did anyone have anything like this? When did it start/ stop? Did you treat it with anything (not looking for product for baby, but natural suggestions would be helpful!)

Baby acne.. warm water with apple cider vinegar wash at the end of his bath, pat him dry and apply organic coconut oil

From all the things i read online. There shouldn’t be such severe issues with babies especially if they are being taken care of. 
My niece had issues with a leaky gut and ascites in liver, doctors keep recommending pharmaceutical drugs but we are hesitant to have her tiny body deal with such heavy doses of medicines.
