It’s possible but it’s actually pretty rare these days.

Yes. I know someone who only saw one at the first ultrasound, saw two at the second, three at the third, then four at the fourth. Luckily it stopped at quadruplets lol. 
It's possible for them to be hiding behind each other.

Yes this happened to my sister, missed at 8 weeks and seen at 12 weeks

There are some people who don’t find out there are 2 babies because one is hiding. I just saw today were someone is 20 weeks and they just found the 2nd baby
