Do you end up with toilet paper bits in your pussy or asshole everytime you wipe?

Every time I wipe I notice I end up with bits of toilet paper. It doesn’t really bother me as I don’t feel it but It bothers me that I can see it. As of recently I’ve noticed I’ve began to itch down there. I’ve tried my best to not itch it but I cannot help myself. So I rub aggressively through my underwear but sometimes I do use my fingers and actually itch it. The itch is not inside my vagina. More near the skin and where the pink area starts. And it’s more near the top above the clit. Should I worry about this? Anything I can do without making a trip to the doctors? Usually before when I get itchy down there it goes away.
