Baby sleep routine

I’ve tried to get a routine established. She’s almost three weeks old now and just went through a cluster feeding phase. Still feeding often sometimes but usually sleeps well in the morning. The past few days she’s been up for HOURS at a time and I worry for her. I know she’s so exhausted and needs to sleep to grow. I feel like I’m not producing enough milk anymore and was told to triple feed (breast then pump a bottle for her) to make sure she gains more weight. I think it’s reduced my supply though. Everything is extra tough being sleep deprived so I have no idea what I’m doing and getting more frustrated. At night we try to bathe or clean her up and put her in pjs then feed and swaddle to sleep. She’s been refusing to sleep when we try to put her down though. It’s worse at night but sometimes she will end up sleeping at least three hours and I’ll have to wake her for feedings. Which is tough for her to transition out of cuz she’s so exhausted. Last night I was so tired I imagined or dreamt I got up for a feeding and didn’t so she went about 6 hours without. I feel absolutely terrible for neglecting her. It’s not the first time I thought I did something for her and didnt. My husband can’t help at night because he has to work and I don’t have any extra milk. Also she refuses to have her arms swaddled but is much calmer when they are. She breaks them out every single time. We’re all so exhausted over here please give us tips on what to do. It’s our first baby and we just want the best for her
