3 month old: sleep issue

Let me start off by saying I KNOW that 3 month old should not be sleeping through the night. My concern is that my son wakes up 3-4 times a night grunting and pushing his legs up on the air. I don’t think he’s hungry but nothing gets him back to sleep lol except a bottle and I don’t want to keep feeding him when he’s not hungry. He’s never been able to take a pacifier. He had a lip and tongue surgery done at 2 months old. I think he struggles to keep suction on it but also he sucks it maybe twice and then tries to push it out with his tongue.
