UK - what's a managed move?

My property isn't suitable for my children or me and my partner. 
My mental health worker is calling the council next week to ask for a "managed move" I don't know what this means.
I forgot to ask him as my brain started talking about other things like I always do!! Any advice?

I used to work in housing. I don’t know if there’s different meanings for the same term but basically it means you will be directly moved to another property rather than going through homeseekers and bidding. I’ve been out the game for a while now though

A managed move is usually requesting a move to more suitable accommodation due to specific reasons such as domestic violence, physical or mental health needs etc.  Your mental health practitioner will likely submit a request to the housing team outlining the reasons behind the move request - i.e on medical grounds due to the impact your current accommodation is having on your mental health and what would be needed to improve your mental health and well-being.
