That looks infected for sure. My daughter had something similar and needed oral and topical antibiotics to get rid of. Actually if I remember correctly they were pretty horrified when they saw her and said it was a good thing I brought her in. Her eczema patch was infected and then she got another cut and that cut became infected without her touching her and looked like what's on your child currently
