I know this is gross but please help!

So I just ended my period 3 days ago and ever since haven't been feeling good! Today I went to the bathroom and I was bleeding bright red blood from my vagina and so I put another tampon in. After two hours of leaving it in I pull it out to see this! Any one know what happened?
