It's how much your cervix has thinned.

Effaced means how thin the cervix is- when you are first pregnant the cervix is thick to maintain the pregnancy. As you get closer to labor it may start to thin out- at delivery you will be 100% effaced or thinned out. Most people worry about how far dilated they are but typically a thick cervix won't dilate- so if you hear that your cervix is hard or moderate it won't efface and typically has a harder time dilating- soft cervix is a labor cervix and should allow you to dilate easier!
