Painful sex

Hi, I've been having sex with my boyfriend for about 9 months now (I'm 17 years old) and it hurts every single time and I feel like I need to pee as well. But it's hurting at the entrance the most. Everyone's said we haven't done enough foreplay but we do plenty and we've tried different positions and lube and with and without a condom. Please help???????
