Bleeding?

Every time my boyfriend fingers me, I bleed. And it's kind of a lot. Some people think it's because his nails probably scratched the walls of my vagina. But he's nails are really short and there's no way that could be the case. We just had sex and I bled a lot too. I've lost my virginity a long time ago so it can't be because my hymen hasn't fully popped yet. We've had sex many times and until recently I've been bleeding a lot whenever I have sex or he fingers me. Is this some kind of condition? Why do I always bleed? It's definitely not my period or spotting. It's pretty embarrassing to bleed each time we have sex. I'm not on any form or birth control other than when we use condoms. Someone please tell me if this is an actual condition or somethings just wrong. I'm definitely going to be seeing a doctor soon.
