Dermal piercing help?!:( (picture included)

I have a dermal piercing in my chest, and last  night I managed to pull the gem off by accident leaving the little bit sticking out of my skin😰 do they have to remove the whole thing to fix this? Or can they just screw in a new little gem? I'm scared too go because I don't want it done again.. It hurt so so much haha. (Had this done over 2 years ago and this is the first time it's gone wrong!
