I feel like he doesn't love me

I have lupus and sometimes it can get really bad my fiancé of 3 years (we've been together for a total of 6 years) is not supportive. Never asks if I need anything, how I'm feeling. Never gives me a hug or kiss without me asking for one. He's all in his own world and I feel alone. He says that he loves me but I just don't feel it. I'm not asking him to be my caretaker but just so general courtesy. He shows more companionship towards the dogs.
