 if you're worried about getting pregnant.. yes the fact that the condom came off means you had unprotected sex. Which can lead to pregnancy. 
2) wtf do you mean there's like a condom up your hoo-ha somewhere? I highly recommend finding it. I mean luckily it can't go too far. But it isn't good to have a foreign object just sitting in there. So I recommend getting it out asap before you get an infection or something.
