Sick nanny 😫 TMI-ish

I'm a nanny for my mom's best friend's grandson. He's 7mo.

I have him right now, until 6pm (it's 9:30am here).

I recently started taking new medication that has made me REALLY nauseated for weeks! But I hadn't thrown up until this morning, about 20 minutes ago. I threw up so violently that a blood vessel in my eye burst and I lost vision for a few seconds.

I know it's because of my meds, so I'm not contagious, but I'm wondering if I should call his Grandma and let her know. I don't want to make her leave work, but I also wouldn't want her to be upset if I didn't tell her.

What should I do?
