hcg levels 48 hours

So I have high dish pg and have gotten my hcg tested and first one was 217, second was 494 third was 1254 the midwife said could be twins cuz the levels are rising significantly the last was was 157% increase in 48 hours!! What are your thoughts im only 4w2d according to my last AF.
