Can't Orgasm?!

Why is it that I can never seem to orgasm? Whether it's having sex or masturbating it just doesn't happen, it's like I'll get to a point where it feels like I'm about to but then it gets too sensitive/intense that it feels painful and I have to stop whatever I'm doing. Am I the only one this happens to?:(
