bleeding first trimester

This is my first pregnancy, I'm almost 12 weeks, and I've pretty much bled the whole time,sometimes just spotting and others full on bleeding, I've been to the hospital and baby is fine every time, they just can't seem to figure out what it is? Has anyone else had this? I have a bruised ovary which is expected to give me cramps but not bleed and they discovered a 'cervical lesion' I'm just so confused because everytime I bleed I still panic 😔😔
