SOS NEED GYNO ADVICE

Okay so I'm 17 and I'm sexually active but my mom doesn't know and she'd be really mad if she knew. I have my first gyno appointment tomorrow and my mom is taking me and she will definitely be reading over my shoulder when I fill out the paperwork. What do I do??? Would it be okay if I said I'm not sexually active on my paperwork and then told the doctor that I am???? Please help I'm freaking out!!!
