What is a case manager?

I just got home state health plan. I got a letter in the mail saying that a case manager wanted to assist me in coordinating my care. I have no clue what this means as I had home state with my last baby and never got contacted by a case manager. I tried calling but no answer. What does this mean?

They help you navigate the health care system and resources available, referrals to other health care professionals, etc.

A case manager is someone who simply takes on your file and makes everything the way it's suppose to be and if you need help they'll help you and you won't get lost and get a random person every time you need info on something so you'll have someone who knows everything in your file from the get go.
