Questions to ask at the first prenatal visit?

Hi! So my first appointment is tomorrow and I'm not sure the OB I'm visiting will be the doctor who delivers my baby. What questions are you guys asking your doctors? Some of mine are: 
1. How long have you been in practice?
2. Are any of the nurses on staff certified nurse midwives?
3. How do you feel about forcep deliverer and in what case would you have to do that?
4. Have you had to use any other intervention besides a c section to deliver?
5. Is there a time limit on how long I should labor?

What are you guys asking your doctor?

My doctor is a certified nurse midwife so I saw her for OB appointments and will see her through pregnancy and postpartum. I know her pretty well but my list of questions are:

What's your stance on pitocin or other medications? 
What about vacuum/forceps?
Do you have a preference in delayed cord clamping (up to five minutes)?
What can I do to prepare my body against tearing during delivery?
After my water breaks when should I call you?
Have you worked with a doula before?
What do you recommend for additional vitamins during my pregnancy?
Do you have any books you would recommend for pregnancy and first weeks of taking care of baby?
