Severe clogged ducts that I can't get out for a week now!

I'm at my wits end as both my breast have two massive lime size clogged ducts and I can't get them out with heat or massage or baby :(

I have tried dangle feeding and pumping with my medela symphony and massage and heat and baths.. hand expressing.

Toothbrush vibrations..most of both breast are giant clogs but I found two tiny knots near the nipple that I think are the clogs so I think the large sac is the backed up milk.

When I express do I hand express as normal of do I press on the clog while expressing?

How do you get them out? I started lecithin today and hope it helps in the future.

I'm so scared it's gonna turn to mastitis as it's been a week. How should I be trying to get them out? Do I press on the tiny knot which seems to be the actual clog or the huge lump? Do I press while expressing or just express?

With dangle feeding how the hell do you latch baby? My breast aren't big enough to touch her face from that position.. help.

I'm crying everyday over this and I can't take it anymore.

I'm looking at surgical drainage if I can't get these out..
