Yeast infection won't go away, Help!!!

This has been the year of yeast infections for me 😓 I used to never get them and now I've had 4-5 this year alone. I think this time I got it from taking antibiotics for my wisdom teeth. My doctor gave me this single does of a pill to cure the infection and two months ago it worked within two days .....this time it's been 4 days and I'm still itchy! When I wipe myself after peeing I also see little bits of skin on the toilet paper as if I rubbed myself Raw down there 🤢 I've also been using the monistate itch cream but as soon as it rubs off I'm itchy again. I changed soaps, laundry soaps, toilet paper, underwear, even drink a ton of water and nothings helping please help :(
