Mirena IUD

I've had my mirena for about a year and a half now. I got it after I had my son. So I haven't had a period at all since getting pregnant with my son in 2015. I have period type symptoms once a month and I call it my fake period. But today, out of nowhere, I'm having bad back pain, I'm nauseas, and when I peed there was a little blood when I wiped. Not a lot, just enough to look pinkish on the toilet paper. Anybody else have this issue? Probably just my period? Or should I be worried?
