I’m so self conscious help

I have a large labia and I’m really self conscious about it, it’s gotten to the point where my bf I’d over two and a half years wants to have sex or even foreplay but I don’t want to because I don’t know how he’s react. I’ll turn him down and although he doesn’t say I can tell he’s upset and I don’t want it to ruin our relationship. I want to get surgery but again I’m too self conscious to speak to a parent or a doctor about it and it’s really taken a toll on my self esteem. What do I do?
