Should I switch providers?

We've had a lot of issues with my OB office. Medicine in my chart that I've never been prescribed. I had my Maternity 21 test done, 3 days later when I went for my appointment they asked if I wanted the blood work, which I guess was not documented. Then I got results a week later. (My husband is nervous it's not mine) Last week I went in for a UTI. They told me everything was normal, a week later they called to tell me there was bacteria in my urine and theres been a lot of little things as well. Am I over reacting or would you be nervous to go there? Should I switch? Would would you do?
