What do I count as the “start”?

So my husband and I started ttc in November and got pregnant pretty fast. I’d just come off birth control and wasn’t expecting a period so I didn’t find out I was pregnant until January. Unfortunately, I had a miscarriage with a D&C on 2/5/18. So my question would be, do I count from November as ttc (9 months ttc now) or from when we officially started ttc again in April (4 months ttc now)? I’m just confused if I count the time before. Mostly wanting to know so I know when I should start thinking about seeing a fertility doctor or talking more with my OB about it. I know it can take a healthy couple up to a year to conceive. Sorry this was so long!!
