Swollen lymph node for YEARS

When my nephew was around 3 years old we noticed he had a really swollen lymph node on one side his neck, when his yearly appt came around a few months later we asked about it and they said it was normal and nothing to worry about. Since they said not to worry we kind of just stopped thinking about it, but it has always been there. Fast forward 5 years, I picked him up from school last week and he was complaining of really bad neck pain and tightness. He couldn't even look up or to the right which is the side with that swollen node.

I'm wondering how is it normal for him to have had this almost his entire life. And can it effect his range of motion for his neck? Anybody have any ideas? Should he go to the doctor?
