Weaning 10month old from on demand b/feeding

Hi-
I have a 10 month old daughter who breastfeeds on demand but no more than three hours during the day and 5at night. A little history... My daughter was diagnosed with a metabolic disease called MCADD. This was found during her newborn screening and only 1 in 15,000 babies get it. She received a mutation in her DNA from her father and I resulting in her not being able to make a certain enzyme in her liver. There for she can not store medium chain fatty acids as a reserve during times of fasting. She can not fast more than six hours and during illness it's 3hrs for the rest of her life. If this does not happen.. She has to be admitted with iv d10 (glucose) until she can keep food down or tolerate it within those times. 
It's been a whirlwind for sure. But now I'm seeing we might need to consider weaning. She never used to have issues but now she bites (four teeth), takes less milk, less interested and the list continues. Since I breastfeed on demand vs a strict schedule.. I'm not sure how to wean her. She of course can't be weaned from night time feelings but can go up to six hours.. She of course won't go past four because of waking to be fed or comfort nursing. I have a hard time telling her no as her medical condition makes it difficult. I need her to stay full or we could loose her in a hypoglycemic crisis. 
I know my situation is a lil different. I also want to say that I don't pump because I rarely get milk from it (can't let down on pump).. And she eats three solid meals (what we eat).. Plus snack, juice and water. 
Bottom line... For on demand nursers ... How did you wean??
