My infertility story (long)

I have 2 children from a previous marriage, they are 10 & 13 and were born when i was 22 & 20, respectively. I'm about to turn 33. I've had no abortions, still births or miscarriages. I had no trouble conceiving those two and within 3 months of unprotected sex. Both were born by c section. I had Mirena for 8 years. I had the second one removed early, in October 2012, in order to start ttc.
My husband has never fathered a child. He will be 33 in a couple months. Never a "pregnancy scare" with any exes either. He has had two semenalysis, which my obgyn reviewed, but haven't been reviewed by anyone else. My obgyn didn't see anything concerning, but also that's not his area of expertise. My husband was checked physically as well and Dr thought he may have a Vericocele issue, but wasn't certain.
I've tracked my cycles since November 2012, including BBT, CM and OPK. I have a fairly normal cycle, average 29 days, and i ovulate on schedule/regularly. Due to ttc without success for over a year, my obgyn decided to get a bit more aggressive. We tried several rounds of clomid, and with each failed round, he increased the dosage. All it did was cause severe ovulation pain and ruptured cysts. He then did ONE ultrasound, and thought that my ovaries appeared to be polycystic, so diagnosed PCOS. (I'm not confident of this diagnosis however, as i do not present with most of the common signs or symptoms of PCOS) In March of 2014 i had an endometrial laparoscopy. He said he found only mild endometriosis, but was concerned that he wasn't able to explore my left tube during the procedure. So I had a dye test about 2 months later. It flushed a mucous plug but showed that my ovaries and tubes were clear. So then he reverted back to clomid, at 150mg. We did one round of that, i had a ruptured cyst and severe, debilitating pain, and refused to do any more.
That was around June 2014.
Since then we've not really actively ttc. We recently started more aggressively tracking and using more <a href="https://play.google.com/store/apps/details?id=com.glow.android">fertility apps</a>. This past cycle, we tried Preseed lubricant and believe that I may have experienced a "chemical pregnancy". 7 days past ovulation i experienced severe prolonged dizziness and nausea. I took an hpt and it had a faint positive line. Continued to for the next two days, then all tests after that were negative. My period came 3-4 days late, which it never is late, always comes on projected date. and the second day of menses i had some severe cramping followed by passing what appeared to be about a silver dollar sized, clotty, mucous membrane.
I got my positive OPK today so we are now in the phase of BD and then the TWW.
I am no longer seeing that obgyn, but haven't located a new one yet. He's close to retirement, and i feel like his methods are a bit old school by the book, and i feel i may have been misdiagnosed. and we've wasted 3 years and thousands of dollars on inadequate, ineffective approaches. If we don't conceive this cycle, we are going to look into more of a fertility specialist to help us.
Aside from Fertility related issues, I also have chronic migraines and chronic vitamin D insufficiency, and I'm overweight but not obese. In all other ways I'm perfectly healthy.
