First fertility appointment. Questions to ask?!

We have our first appointment at our new fertility clinic tomorrow. We have male factor infertility and were told our only option was IVF / ICSI. I’m assuming he will need to get another SA since its been about 4 months since his last one. Luckily our insurance covers infertility treatment but it says we need to do a years worth of IUI before they approve IVF but they also said if IUI will not work, the doctor can write a letter of medical necessity. Any ideas on what questions we should ask? I’m so nervous to get this process started!
