Hanging toys or adding anything that didn’t come with the car seat is unsafe. They are not crash tested with your car seat so they should be removed when in the car and in motion.

No, I’ve never put a toy on the car seat. Most car seats explicitly say not to add or attach anything to the seat that didn’t come with it. 
Remember in the even of an accident, even a very minor one, that can become a projectile. If you wouldn’t throw it at your baby fill force, leave it out. We opt for soft toys or a plush when they’re older.
