Extremely sensitive perineum

Okay so way before I got pregnant, I would have sex often and be very sore down there and i’d have small tears on my perineum. well I still get them and I haven’t had sex in a few weeks so i know it isnt just sex related. I think my skin is just very dry down there? or maybe it’s how I wipe when I use toilet paper? I have very sensitive skin down there and am very prone to infection. it happens every now and again and I have to be very gentle down there so it doesn’t turn into an infection. how can I prevent this? should I bring it up with my doctor?
