Condom hurts me so bad

Lately I have been experiencing some difficulties when having sex. I'd ask my partner to put on a condom everytime, and the second he penetrated in my vagina, I got an excruciating pain. Everytime he moved, it felt like the wall inside my vagina was being ripped. This had never happened before I tried different condoms and lube. The condoms that we normally used and liked (the ultra thin ones) also hurt me though it never happened before. This started about 4 months ago.

Has anyone experienced this and/or know what happens to me?
