No. What we need is better understanding of consent and more respect for each other (particularly for women, our humanity, and our rights to our bodies). 
We should be raising our children to seek enthusiastic consent from their partners, communicate openly about these things beforehand, and communicate and listen in the moment. We need to change a culture that objectifies women’s bodies as only sexual objects to be pursued and “won” or conquered. We need to stop with tropes like “good girls” have to be convinced or pushed to do sexual things, or that men have to pursue women/should like a “challenge,” or that talking about things is not sexy. We need to change toxic masculinity and male entitlement. 
What we need is cultural change regarding sex, our perceptions of sex, and the treatment of everyone who isn’t a straight in this country. We do not need forms that are useless, because as someone else noted, consent can be revoked or changed at anytime, even mid sex. 
We need respect and bodily autonomy. Not useless bullshit.

A simple yes, no, or, stop should suffice
