When I had mine done, it was both. But opposite from the previous commenter. I had a full bladder for the abdominal, then I got to relieve myself for the transvaginal.

I work in radiology, when a doctor orders a complete they do both abdominal and transvaginal. You will most likely need to have a full bladder for the transvaginal part then they let you relieve yourself and proceed with the abdominal portion.
