Should I take him to the ER?

So my 2 and a half year old has had a very high temperature for about 3 days now. High as in 39.5 Celsius. his body is very hot to touch. I’ve given him ibuprofen and paracetamol and nothing really helps.. he’s had no other symptoms except diarrhoea for a couple days. He keeps pointing to his belly and saying it’s in pain. Does anyone know what this is? Should I take him ER? He’s also jumping in his sleep a lot..
