Sore nipples anyone?

So my little man is 5 weeks today. He is EBF and my lord my nipples are still so sore. I can't even wear a bra for very long and the breastpads are the worst with squishing my nipples and making them itchy. The slightest brush against them is awful. breastfeeding has gotten a lot better with us both getting more skilled and a better latch. I have tried the Medela nipple shields but it just cuts into my skin and is almost worse then just the cloth. I have loose, soft T shirts that are decent but then I struggle with leaking everywhere and definitely can't go out like that to the store. I was wondering if anyone is dealing with this still? What do you wear at home or when you go out?
