99.4 armpit temperature 99.6 rectal temperature help

I know 99.4 and above on armpit is considered a fever. So I took his temperature rectal and it was 99.6.

He is 4 weeks old and was in the Nicu for 7 day antibiotics due to a bacterial infection. Should I take him to the Er?
