Mamas of reflux babies!

This might be a little long as I explain my situation so please bear with me. I need help. My son was diagnosed with severe silent reflux a little over a month ago. He was struggling terribly to eat - would arch, cry, pull away from the bottle but wasn’t spitting up. His doctor started him on ranitidine. It took awhile to help but eventually kicked in. They also had me switch to Nutramigen since it would be easier on his tummy. The combination seemed to help quite a bit and we were doing good. We ran low on the ranitidine about a week before the Thanksgiving break and the pharmacy was all out - it was on back order. I was just able to get more yesterday, so he went 4 days without it. By the time I got his refill yesterday I could definitely tell he still needed it.

Now - my question is related to sleep. I haven’t been able to get him to sleep on his own in the pack n play hardly at all. I thought it was related to swaddling. He hates being swaddled and I was struggling to find an alternative that gave him freedom with his arms but wouldn’t allow him to startle himself. No matter what I tried it would always be the same thing...constant wiggling, grunting, groaning until he’d end up waking himself up. I had been chalking this up to having his hands and arms semi contained and assumed he was wanting to get free. So when he’d wake himself I’d pick him up and just let him sleep on my chest and he’d sleep for hours without moving at all...even with his arms contained.

Well - being off the medicine for 4 days he got pretty miserable. He was struggling to eat again but the biggest thing I noticed....he was exhibiting the same behavior he does sleeping in his pack n play but much more frequently. He would wiggle, grunt, and groan even when sleeping on my chest or napping in his car seat while out at the store. Even did this during diaper changes. So now I’m starting to think his aversion to sleeping in his pack n play is from his reflux. I’m guessing the medication suppresses it enough that he’s fine while awake or when sleeping on his belly on my chest, but when he’s laying flat on his back it must bother him.

What would you ladies say? Does that sound like it could be it? What can I do to help him so he can lay on his back and not be uncomfortable? I’m not sure how to elevate the head of the pack n play. Should I take him to his doctor and see if they maybe need to try a different antacid? I’m at a bit of a loss.
