Looks like yeast to me. My daughter used to get rashes like that too. Doctor prescribed Nyststin and it was gone within two days. Now after we switched wipes and diapers and dry her off after we wipe her we haven't had any issues. We also use butt paste in the red tube or jar as a preventative before bed

You need to get a prescription cream from your doctor, that’s going to be painful for baby. Mine had this from her formula and we battled on and off for 9 months(definitely try to find out what caused it because often it’s an issue in the digestive system causing acidic poop), I will tell you once you get the prescription cream it will go away quick- like one or two days, as crazy as it seems. You just need to pulll a dryer out and air dry bottom after every single diaper(this is the crucial step), use the cream and then change immediately when they pee or poop agin. It’s so annoying but after one day you will see a huge improvement. Best of luck!

Poor baby 😔 like someone else says, it looks wet. Warm bath with no perfumed soaps. Then lightly PAT dry and do not put on a diaper for 30 minutes. Let the rash get a good air dry. 
That looks painful, so I would also ask the doctor for suggestions on how to treat it and how to prevent it from happening again. You nat need to change diaper/wipes
