Help meee pleasee

I’ve been at my job for a month (worked there before a year ago but different manager and everyone) ... my boyfriend ended up getting an apt and wants me to move. He live an hour away and we have two kids.. im scared to tell my boss, I don’t know what to even say because so many other people left cause they’re going on with their live doing things... and has told me but I never knew I was moving yet my boyfriend surprised me about the apt... how do I tell her?? I want to tell her face to face next time she works.. I’m just so nervous cause I’ve only been there a month..
