HR makes me want to quit rant!

So I became eligible for FMLA on 1/7 and so I went to talk to my hr benefits rep to set it up. She handed me a paper and said that is how you set it up. You can have 4 weeks for a natural birth and six weeks for cesarean and sent me out the door. She didn't explain how to set it up or how it works backdating it for intermittent leave to cover doctor appointments. I went for another meeting because the paper made no sense and she talked down to me condescending way like I was stupid and told me just call the number on the paper. She then went on asking me why im already having appointments 2-3 weeks apart asking me if I am high risk and what problems im having and when my due date is. FMLA is outsourced to another company so it's none of her business. I call the number on the paper which only talks about their online portal for the st and lt leave not intermittent. I get intermittent set up because I just don't have the PTO for my appointments and the next day I get a call on my work phone from the higher HR rep being I got a form from your HR rep saying that your leave started yesterday for short term disability and I just wanted to check because usually, that doesn't start until a month before rather than now clear through the end of the pregnancy and your back the next day after the baby is born and I had to tell her to talk to the other hr rep because she screwed up with whatever she had to fill out because yes it is only intermittent at this point. Then yesterday I get a message from my team lead saying that since my FMLA isn't there that I will get another occurrence putting me on a verbal for taking today off for my doctor appointment. Before being eligible for the FMLA I had my doctor fill out a ADA form excusing any absence related to seeing him. My HR rep claimed she never got it. He sent it twice and I sent it once already. I emailed it to her again cc her boss and boom she magically has it but now she won't process it because my FMLA which won't backtrack to all of my appointments will be active next week. My current team lead im only with as a trial and can't be hired on at that team's pay (it's a level 4 team im currently level 2) if im on any verbal or written communication. My team lead will be on maternity leave when it would roll-off. She wants to put me on her team fully by Monday except now she can't. My HR rep is making me lose out on a 34% pay raise for two months and still can't do her job as she is supposed to 😠 I want to go above her head to corporate. When my team lead tried discussing with her the ADA form our HR rep didn't want to answer her questions and wanted to just talk to her about her pregnancy:P
