Dr. Appointment help needed!

Hoping to get some help and advice from all of you.
I have a doctors appointment this afternoon with a new gynecologist for my physical and also to address fertility.
I’ve had period issues/irregularity since 9th grade. I am currently 27. I have been on about 4 different pills, the pill that makes you only have your period every three months, a patch and an IUD. I started birth control for my irregular long and heavy periods. Throughout these years I have struggled with very long periods, very heavy periods, terrible breast pain every month and spotting which takes place a week or more before my period comes. Every time I changed my BC was due to symptoms I was experiencing at the time and recommendations from my gynecologist. My body even rejected my IUD, it was terrible.
I have been off all types of Birth control for about a year. Every month I still get terrible breast pain to the point it hurts to sit up, lay on my stomach and workout. I also spot about a week (sometimes more) before I get my period. It’s dark brown and light but still ruins my underwear at times (very variable).
I am a nurse, so of course I think I might have hormonal issues? Low progesterone? PCOS?
Have any of you experienced this? What questions should I ask at my appointment?
