Any recommendation on Melasma treatment?

My self confidence got affected when my severe melasma on my cheecks appeared when I turned 45. I am 48 years old now and haven’t found the best remedy for my melasma despite the huge amount of money I spent to famous cosmetic syrgein and dermatologist in town. I have done lasers, high-end /local facial creams  and different kinds of Skin peeling but none worked. I am taking my thyrax pills for as my lifetime thyroid meds (I got my full thyroid removed due to cancer).

Hi I worked in aesthetic&plastic medicine for a long time and we saw a lot of melasma. Have you gone and had a dermatologist determine what triggered it? That would be the place to start if you haven't done that yet. Sometimes if it is hormonal and the exact hormonal imbalance is pinpointed and corrected it can help clear it up. For daily management/help here are a few tips that can work for some but ad melasma is tricky unfortunatly they may just stop it from getting worse but maybe not help decrease it. First off AVOID THE SUN!!! Biggest one. Try to make sure your face is always protected as sun exposure can increase melasma discoloration. Hats that block are great but also find a good sunscreen that is a non chemical physical blocker sun screen. So one that uses things like zinc as opposed to a chemical skin protectant. The non chemical.ones protect the skin much better from harmful rays that can trigger discoloration. Microneedling has a good effect on some people for evening out the discoloration plus just makes your skin look good. A retinoid/retinol cream can help too but make sure it is high quality...dermatologist should be able to suggest or prescribe one. Using a vitamin c or e oil/serum at night can also help. Those vitamins help heal skin and correct sun damage. Again find one of good quality with few weird ingrediants. There are lightening creams that a derm can prescribe also but they can be very harsh on skin, have side effects, and also may not give great results so be wary and ask lots of questions with any skin lightening cream. And biggest thing is continuous maintenance/prevention. If you can get it under control with a good regime literally 1 sunburn on the face can trigger it again so be vigilant. Laser and light treatments are highly ineffective and cost a lot so. I would avoid them and avoid BBL in general. BBL machines are great for sun spots, freckles and general discoloration but should not treat melasma. It can make it much worse. Melasma is hard since it is just cosmetic not a lot has come out to help it. There are some new oral meds but they haven't been out and used widely enough for me to have seen the effect but I hope they are a step in the right direction. Good luck!!
