Incision Opened 3 months pp?

Is it possible to have a spot of my incision open up at 3 months pp?

It was healed and I was cleared to resume all normal activities at 6 weeks and haven't had an issue until now. I thought it was just irritation, but a purple spot almost like a blood blister popped up on right side and it opened up yesterday. Not sure what to do. OB is a 2 hr drive, but I have a clinic local I can see.
