Terrible sleepler help!!

My toddler still BFs during nap and all night. Every hour L.O demands to have the breast at night. We cosleep. I have tried sleep training in her crib at 12 months. She will not sleep no matter how tired and because of the ST she's now afraid of the dark alone. I want to start weaning but don't know how to approach or start. She likes her crib and says sleep when she's playing but won't settle for actual sleep. I'm so over breastfeeding please help!!!
