remember a nurse asked me for a notarized paper for something I don't remember

Please just remember a birth plan is an outline of what you want to happen. It doesn't mean that's what will happen. Sometimes things need to be changed last minute.

No just fill it out yourself and give it to the delivery team on the day of your labor. You can show it to the OB or midwife you’re seeing before birth too to go over it together
