I do mine at pre-registration with my hospital. But my doctor lets me do what I want and I can change anything at any time with no questions asked.

My hospital had you submit it when you did pre-registration. Mine was only 3 sentences so my ob already knew haha. But where I am you give it early.

I brought it to my OB appointment at 32 weeks and went over it with my doctor so he knew my desires in advance. I then packed it in my hospital bag to bring for the nurses.
