Looks like a milk bleb. I had one from 3 months pp to maybe 5 months. It hurt I made sure to hold the shower head over my boob for a bit vand I used a hot compress before I nursed.

How old is your baby? Your nipple could be raw. It happens to me a lot especially in the beginning
